I've had a bbPress plugin author look at the code since the original author isn't supporting any of their bbPress plugins anymore and he had no problem getting it to work - and apparently on a Windows server (which is what we are on).
I haven't given up on trying to find a solution but it may just take a little longer than I thought.

Just a stupid thought… Have you had a look at the permissions on the attachment folder(s)? If the folder is accessed through a share, are the permissions on the share set correctly? I recently had a similar problem, I had forgotten to modify the share's permissions

On an IIS server?
IIRC IIS folders are natively set to 777 or similar ?
Mike would be the one to ask about that

Sorry about the delay. I was referring to the NTFS permissions on the actual disk locations, not the ones on the IIS virtual folders (although you should also check those, just in case).
